The Relocation Services Department contracts with designated Managed Care Organizations (MCO's) and several community organizations to help people transition to the community. The Relocation Services Department with CBFL would like to share information with surrounding nursing facilities to understand the program and how to provide referrals from their facility. This informational will allow discharge options for nursing facility residents based upon their eligibility.
